Comparison of treatment methods for varicose veins

Below is a clear comparison of the three main procedures: traditional vein stripping surgery, endoluminal thermal ablation (laser or radiofrequency) and VenaSeal®.

Vein stripping surgery

  • Type of procedure: Surgical removal of the vein
  • Anesthesia: General or spinal anesthesia
  • Setting: Operating room, sometimes short inpatient stay
  • Tissue trauma: Very high
  • Pain: Moderate to severe
  • Scarring: Multiple incisions
  • Nerve injury: Increased risk
  • Compression: Required
  • Return to work: Days to weeks
  • Costs: Covered by health insurance ✔

Endoluminal thermal ablation
(laser / radiofrequency)

  • Type of procedure: Minimally invasive using heat
  • Anesthesia: Tumescent local anesthesia, often with sedation
  • Duration: approx. 1.5 hours
  • Tissue trauma: Moderate
  • Pain: Mild to moderate
  • Scarring: Small puncture sites
  • Nerve injury: Possible due to heat
  • Compression: Not required
  • Return to work: Usually within a few days
  • Costs: Covered by health insurance ✔

VenaSeal®
(medical adhesive)

  • Type of procedure: Minimally invasive using medical adhesive
  • Anesthesia: Local anesthesia at the puncture site
  • Duration: approx. 1 hour
  • Tissue trauma: Minimal
  • Pain: Minimal to none
  • Scarring: Tiny puncture site
  • Nerve injury: Extremely rare
  • Compression: Not required
  • Return to work: Usually immediate
  • Costs: Self-pay, approx. CHF 2000.00

Which is the best and most gentle vein treatment method?

Modern treatment of varicose veins follows a clear principle: as gentle as possible, as effective as necessary. The choice depends on medical findings, stage of disease, anatomy, and the individual preferences of the patient.

Based on current venous medicine, the main methods can be ranked in terms of gentleness, comfort, and long-term effectiveness as follows:

No. 1: VenaSeal® – maximum gentleness, no heat

The VenaSeal® procedure is currently one of the gentlest methods for treating saphenous veins. A medical adhesive is delivered into the vein via a catheter, permanently sealing it from the inside – without heat, without tumescent anesthesia, and without general anesthesia.

Advantages:

  • No thermal damage to surrounding tissue
  • Usually no compression stockings required
  • Minimally invasive and outpatient
  • Very fast return to daily activities
  • Minimal bruising

This method is highly advanced and particularly comfortable for patients. However, in Switzerland it is generally a self-pay treatment, as health insurance does not usually cover the costs.

No. 2: Endovenous laser therapy – gold standard with long-term data

Endovenous laser therapy has been established for over two decades and is supported by strong scientific evidence. Under ultrasound guidance, a fine laser fiber is inserted into the vein, and targeted heat energy closes it permanently.

Advantages:

  • Minimally invasive
  • Outpatient procedure
  • Covered by health insurance
  • Very high closure rates
  • Long-term studies over 10+ years

With modern 1940 nm laser technology and radial fibers, the procedure is now particularly tissue-friendly. Compared to VenaSeal®, heat is used, but in a controlled and precise manner. Downtime is short – often home office is possible the next day, with a return to normal work shortly thereafter.

No. 3: Hook technique (phlebectomy) – targeted removal of side branches

Phlebectomy is used to remove visible, tortuous side branch varicose veins. Through tiny 1–2 mm micro-punctures, the veins are gently extracted using fine hooks.

Advantages:

  • Very small skin openings
  • No large incisions
  • Excellent aesthetic results
  • Outpatient procedure

The hook technique is often combined with laser therapy or VenaSeal® to achieve optimal functional and cosmetic results.

No. 4: Compression therapy – symptom relief, not causal

Compression therapy using medical stockings can reduce symptoms such as swelling and heaviness. However, it does not treat the underlying cause, as the diseased veins remain.

Disadvantages:

  • No cure of varicose veins
  • Lifelong use required
  • Often perceived as inconvenient in daily life

Conclusion

The most gentle method today is often VenaSeal®, especially for patients who wish to avoid thermal treatment.

Endovenous laser therapy remains the proven gold standard covered by insurance, with excellent long-term results.

The hook technique optimally complements modern saphenous vein treatments for visible side branches, while compression therapy serves only as a supportive, non-causal approach.

Decisive for every treatment plan is precise duplex ultrasound diagnostics and an individualized approach based on the latest standards in venous medicine.
